Securing value-based care partnerships in Puerto Rico was a significant achievement toward delivering on our mission. Under the agreements, we will work with providers to integrate care management plans and provide members with access to multidisciplinary care teams, including dietitians and social workers.

These partnerships expanded our managed members to seven consecutive time zones. However, expanding outside the continental U.S. was new for us, so we had to learn more about the culture, language, and way of working. Our knowledge and experience in value-based care would only be valuable if we could translate it to the local market.

This involved working with providers in Puerto Rico to better understand their current care models, systems, and processes, before diving in with solutions. They know their patients best, and by listening first, we were able to better understand the needs and experiences of our partners and patients and how we could support them.

This process took several months and involved teams from across our organization—Legal and Compliance, Business Development, Program Management, Marketing, Account Management, and many others.

Being open to learning allowed us to strengthen our relationships in Puerto Rico, putting us in a better position to improve the lives of members with kidney disease.

As we expand to other locations, we'll apply the processes and lessons learned in Puerto Rico to adapt our services to local nuances and needs.
