Even with 40 years of experience as a registered nurse, Amy was not prepared for her own diagnosis of chronic kidney disease. Feeling overwhelmed and uncertain, she put off seeking help.
Fifteen years later, Amy received information from her insurance provider about our program, and it finally made her feel like managing kidney disease was possible – perhaps even easy.
Amy was assigned a care team, including a nurse, dietitian, pharmacist, and social worker, who worked in tandem with her nephrologist. Together, they made sure Amy focused on improving her diet and lifestyle.
And Amy loves that her care team is just one chat away. Interwell’s on-demand care model allows her to reach her team and receive support by phone or in person.
With her team behind her, Amy has more time to take ownership of her health, particularly her diet. Amy is working with an Interwell Dietitian to adopt a Mediterranean-based diet. She uses the recipes available through Interwell’s patient application to make the right food choices, like switching to fresh herbs. Amy also took up biking with her sister as another way to incorporate healthier habits.
“Since I’ve started the program, I haven’t had a single potato chip and haven’t missed it one bit,” Amy said. Amy and her care team have worked hard to slow the progression of her disease and keep her off dialysis. While she hopes to continue that trend, she knows that if dialysis is ultimately needed, she’ll be educated about her options, thanks to our program and her team.
“I am prepared,” Amy said. “I don’t have to wait until I’m in an emergency room with someone else making decisions for me.”
