Partnering Together to Improve Lives

Karen was overwhelmed at the beginning of her CKD journey. Many providers and programs reached out to her, but she didn’t know where to start. Then she got a call from Interwell Health Registered Nurse Molla Perrigo.

In a single phone call, Molla was able to connect Karen to one of our social workers and a pharmacist. All three care team members worked together to address the patient's concerns. Karen especially loved that her team accommodated her schedule and met with her in between her 10-month-old granddaughter's naps.

Behind the scenes, Molla focused on being the single point of contact for Karen—aligning with her providers and gathering answers to all her questions. With Molla’s support, Karen learned how to advocate for herself and take control of her own health. Our care team also helped Karen decrease her blood pressure and catch an acute kidney injury early on in her journey. We know we are better when we work together, and we are thrilled we were able to make connections across Karen’s care team to help her live a healthier life.
