A Virtual Health Journey

2,280 days and 835 chat messages later, Nurse Practitioner Bianca Blanco’s time with Brian was coming to an end. As one of the first patients in our program, Bianca and Brian created a personal connection through travel—they shared details and stories from their favorite trips.

Bianca supported Brian every step of the way along his journey from providing information on his diagnosis to choosing treatment options, to an IgA recurrence (a kidney disease causing inflammation and kidney tissue damage). Bianca was able to provide consistent, personalized care through the chat feature on our patient application. The patient application offered Brian a place to connect with a community of other patients who were experiencing the same challenges. Brian read articles, learned through FAQs, watched videos, and reached out to Bianca at any moment on our app.

Bianca worked with a holistic care team at Interwell Health—social workers, pharmacists, and dietitians—to help Brian see the bigger picture of his care. They helped him see that his labs, medication, and diet all played a role in his overall health.

Despite his chronic kidney disease diagnosis, dialysis, transplant, and a global pandemic, with support from a united team, Brian now enjoys his retirement and is traveling the world. Our care team, led by Bianca, made sure Brian was doing more of what he loves and spending less time worrying about his health. Bianca is proud that Brian has the tools to continue his journey and knows that he holds a special place for her.
